To obtain your Key Card, please fill out the form below for faster service.
You can  then pick  up your card at our office at 8000 Columbus Road.
Card pick up times are now Monday and Fridays only from 9-11 AM
or call 330-875-6249  for an appointment for another day/time between 7 am - 3 pm.  
Please bring a drivers license or utility bill and picture ID
with you to prove your address.
Cards can also be requested in-person during the above hours at our office.
